A clinical sign to predict difficult tracheal intubation: a prospective study. Can Anaesth Soc J 1985 Jul;32(4):429-434. The Mallampati classification compares the size of the tongue to the size of the pharynx, it is graded from I to IV. There are four things it assesses: uvula, soft palate, fauces, and pillars.
Mallampati I: the entire soft palate, fauces, uvula, anterior and posterior pilars are visible. Correlated with excellent laryngoscopic view.
Mallampati II: two things not visible: the anterior and posterior pillars. Any range of laryngoscopic difficulty.
Mallampati III: fauces and pillars not visible. Only the base of the uvula and the soft palate are visualized. Any range of laryngoscopic difficulty.
Mallampati IV: nothing visible. Cannot see soft palate, uvula, fauces or pillars. Difficult laryngoscopic visualization.
